I evaluated a 44-year-old female wanting to improve her stomach area after massive weight loss. During her consultation, she explained how she was hoping to remove the loose skin as well as contour specific areas of her midsection. After listening to her and setting the expectation, we agreed to schedule two separate procedures to enhance the area.

In September 2016, I performed a panniculectomy, which involves surgically removing the excess skin and adipose tissue. Then in March 2017, I performed liposuction to improve the contour of her stomach/abdominal area.

I evaluated a 28-year-old female that was wanting to have a breast enhancement as well as address her stomach area concerns. After losing over 120 pounds, her breasts were deflated, and her abdomen had excess loose skin. After her examination and listening to her goals, we agreed on the expectation and scheduled her for surgery.

During her surgery, I performed a breast augmentation using high-profile 350 cc saline breast implants along with a formal mastopexy (breast lift). Regarding the stomach area, I performed an abdominoplasty (tummy tuck) to remove and tighten the skin. Performing these procedures after massive weight loss is very gratifying, especially when someone is only 28 years old.

The post-op photo is three months out, and her transformation is well on its way. She is extremely excited about her appearance and looks forward to this summer.

About six months ago, I performed a massive tummy tuck and breast lift on a patient with the intent to have a follow-up surgery to complete her full body contouring makeover. After having three children all by c-sections and undergoing significant weight loss, we agreed to reach her goal by having two surgeries. The second surgery entailed liposuction around the abdominal area and inner thighs as well as removal of excess skin (dog ears) that had bunched up from the tummy tuck procedure.

The first photo to the left was taken right before her first surgery. The middle photo was taken right before her second surgery (liposuction). The far right photo was taken three days post-op after her second surgery. The photo below is now four weeks post op. As you can see, her waist and stomach area look great, and she is extremely happy with her new shape.

BODY SCULPTING TOTAL BODY MAKEOVER

These examples show patients who have had significant fluctuation in their weight associated with pregnancy, breastfeeding and massive weight loss. As a result, they have undergone augmentation with breast lifts or augmentation with tummy tucks and breast lifts. These patients are excellent examples how patients can regain their life and self-image by having tightening procedures of both the breast and abdominal areas to pre-pregnancy status. Patients often present after breastfeeding with significant fluctuation of skin laxity of the breast causing involutional upper pole loss of fullness and atrophy as well as severe skin laxity where the nipple may be greater than 3 cm below the fold, referred to as grade 3 ptosis. These patients will require a combination of implants (saline or silicone) placed under the muscle and a formal mastopexy concurrently with the complete scar of a Wise-pattern. The first example shows a patient who has undergone an augmentation mammoplasty procedure as well as a formal breast lift, a full abdominoplasty and sculpting of the muffin top hip areas.

The next patient shows specifically loss of upper pole fullness without skin laxity and abdominal wall laxity with rectus diastasis. Excellent candidate for straight saline augmentation in the dual plane technique and a full abdominoplasty with plication of midline rectus sheath. Both of these are excellent examples of when patients are placed under general anesthesia with a Board Certified Anesthesiologist and full medical clearance can undergo successfully, in an ambulatory facility, full body contouring and mommy makeover procedures without incident.

Body Makeover After Weight Loss

I evaluated a female who was looking to have a body makeover. During the consultation, she stated she wanted to have her abdomen enhanced, her hips shaped, and a breast augmentation. She explained that after having weight loss, her goal was to get her shape back to where it was in her younger days.

After setting her expectations, we decided to perform an abdominoplasty or tummy tuck along with liposuction of the hips area and a breast augmentation.  For the breast augmentation, I placed 425 cc high-profile silicone breast implants and positioned them in a subglandular position. Subglandular means the implant is placed in a pocket behind the breast gland, entirely on top of the pectoralis major muscle.

As you can see, after only eight weeks post op, the abdomen, hips, and  breasts look fantastic, and she is very pleased with her recovery.

Body Contouring of Abdomen and Breast

Back in July, 2015 I was presented a patient looking to enhance her shape by lifting her breast and contouring the abdomen. During the consultation and examination we detected two weak spots in the abdominal wall, which she stated, she was having pain during certain activities or in certain positions.  We discussed her expectation and agreed that we would proceed with a  Breast Augmentation with lift, tummy tuck and repairing the two hernias.

For the procedure we had a general surgeon repair the hernias, and then I performed the abdominoplasty (tummy tuck)  along with a breast lift and 350cc saline breast implant filled to 370cc to her right breast and 350cc filled to 380cc to the left breast.

As you can see, after only six months post op, the patient is looking fantastic, and I’m very pleased with her recovery.

Mommy Makeover / Tummy Tuck & Breast Lift

I was presented with a 40-year-old female who was looking to have a mommy makeover. During the consultation she was looking to have her abdomen enhanced as well as a breast augmentation. She explained that after 2 hernia surgeries and post pregnancy her goal was to get her shape back to pre pregnancy.

After setting her expectations, we decided to perform an abdominoplasty or tummy tuck, along with a breast lift and 240cc saline breast implant filled to 270cc to her right breast and 350cc to the left breast.

As you can see, after only six weeks post op, the patient is looking fantastic, and I’m very pleased with her recovery.

 

REQUIRED STEPS OF AN ABDOMINOPLASTY

We perform abdominoplasty and tummy tuck procedures weekly in our Beverly Hills Surgery Center as our patients undergoing complete mommy makeover reconstruction, including augmentation, breast enhancement, mastopexy, body lipo contouring and abdominoplasty. The perfect tummy tuck requires multiple steps during the surgical procedure. The first step includes the incision site being made near the inferior position above the suprapubic hairline. This should be placed low enough that the patient will be able to wear postoperatively the clothing and fashion that the scar will be well hidden. The dissection should be carried out carefully meticulously above the fascia of the external oblique muscle midline rectus sheath up to the subcostal ribs maintaining a cuff of fat with the umbilicus to maintains its blood supply, preventing avascular necrosis; however, death of the umbilicus or belly button. The photographs displayed here show a clean dissection anatomically of the entire abdominal wall. Notice the midline rectus fascia and notice the sutures of the O-Ethibond in the midline rectus sheath. This patient is 42 years old and status post deliveries. Notice the external oblique muscle laterally. It’s tight and does not require further tightening sutures. Midline plication has been performed both supraumbilical and infraumbilically from two fingerbreadths below the subxiphoid point to the umbilicus and once again below to the suprapubic region. Plication of the muscle sheath is absolutely essential for a firm and tight appearance to the lower abdominal wall.

The next photograph shows specifically patient in a V-Y configuration, also referred to as the semi-Fowler position with redundant dermoglandular tissue retracted inferiorly, carefully marked and incised as well as allow for the removal of the redundant skin and fatty tissue. After closure of the wound, tumescent liposculpturing the bilateral iliac crest hip region with tumescent liposculpture technique is performed and the reposition of belly button and umbilicus. This ends the final step, which should be done in an oval-like fashion, which will allow for a more natural appearance to the umbilicus rather than round. We used two 10 mm JP drains in order to remove serous fluid as well as small amount of blood as well as tumescent liposuction fluid from the abdominal wall which is furthermore also allows scarring back down in the abdominal tissue to the muscle fascia. Patients are placed with abdominal binders for up to six weeks. Antibiotics were seven days, pain control with Norco. It is important to understand the specific anatomy and the specific pathological diagnosis and physiological sex of pregnancy and significant weight fluctuation on the abdominal wall. The tightening of the abdominal muscle is essential for retraining optimal results and intraoperative photos are taken prior to abdominal wall closure. It should be discussed with your plastic surgeon prior to the operation.

Tummy Tuck and Breast Lift

I was presented with a 36-year-old female who was looking to enhance her abdomen, hips, and breast. During the consultation, we discussed her goals, which included achieving a firmer stomach, contouring her hips, and lifting her breasts. After setting her expectations, we decided to perform an abdominoplasty or tummy tuck along with liposuction to address her hips. Then, to correct her sagging breasts, we performed a full mastopexy or breast lift along with an augmentation using 240cc saline implants.

As you can see, after only six weeks post op, the patient is looking fantastic, and I’m very pleased with her continued recovery.
