Breast Augmentation With Benelli Lift

The female patient presented is 26 years old and was looking to improve her breast appearance by increasing her breast volume and position. However, her thoughts about having a breast lift were concerning because of the potential scarring. During her examination, she showed a mild degree of breast sagging, and based on her expectations, a lift would be needed. After examining and listening to her goals, we agreed to schedule a primary augmentation and lift.

During her surgery, I placed 400 cc saline high profile breast implants. I overfilled the right implant to 440 cc and the left side to 455 cc to create fullness and symmetry. Because the implant alone would not elevate the breast enough and knowing her concern about scarring, I performed an incision type called the Benelli Lift to raise the nipples slightly.

As you can see, the patient is only four weeks post-surgery, but she has nice volume and symmetry, and she has a more youthful appearance.

Breast Revision With Benelli Lift

The female patient presented is 32-years-old looking to remove her over 10-year implants and improve her breast appearance. During her examination, she expressed she was looking to have a lift as well as have an increase in volume. After discussing her expectation, we agreed to replace the old implants and perform a mastopexy (breast lift).

During her surgery, I removed the implants, replaced them with high profile saline 550cc, and overfilled to 640cc. I provided a supportive sling for the implant by maintaining the inframammary (under the breast) fold to preserve a natural breast shape. For the right breast, I performed a Benelli lift or donut mastopexy to raise the nipple slightly.

As you can see, the patient is only a few days post-op, but she has achieved nice volume and symmetrical balance.

NY Times Article “After Mastectomies”

Nytimes article "After Mastectomies"A few days ago on January 29, 2017, New York Times posted an article by Roni Caryn Rabin titled After Mastectomies, an Unexpected Blow: Numb New Breasts. The article presents a few patients’ stories about having mastectomies and the subsequent loss of sensations in their breasts. As the article states, “While doctors agree on the need for a mastectomy procedure that spares nerves, they note that the goal of the surgery is to make sure the cancer is gone.”

As a board-certified plastic surgeon, I believe it is important to share these types of articles so patients can be better informed as they look toward breast reconstructive options.

Rippling and Capsular Contracture, Revision Breast Surgery

breast revision surgeryThe female patient presented to the left was requesting to have a breast revision surgery due to her breast being asymmetry . During the examination, the breast had asymmetry due to the right breast having scar tissue (capsular contracture) and the left breast experiencing a rippling effect. Capsular contracture is scar tissue that forms around the breast implant which causes the breast to harden and the rippling was due to the amount of soft tissue covering the implant.  After listening to her goals, and agreeing on the expectation, we scheduled a surgery date.

During the surgery we used 445 cc SRX extra full profile silicone (gel) implants. This implant has a smooth shell surface and provides the patient with the fullest, roundest look. Using the silicone gel will also help reduce most of the visible rippling.

The post-op photo to the right is at 7-weeks and as you can see her breast are symmetric and she is very pleased with her shape and fullness.

6 Months After Breast Augmentation

Six months after performing a primary breast augmentation surgery for a 50 year old female Latina, it was great to hear her express how happy she is with her surgery. She had come into my office looking to increase her size, shape and cleavage.

To achieve her expectation I selected 320 cc high profile saline breast implants and filled the right breast to 350 cc and 360 cc to the left breast. The incision for the augmentation was performed under the areola and the implants where placed behind the muscle, to reduce the risk of scar tissue capsular contracture and palpability of the implant.

The post op photo to the left is six months out and she has a excellent look and balance.

I had the privilege to see the patient at eight months post op and as you can see by the photo below she is doing very well.

37 Year Old Female With A Ruptured Implant

A 37-year-old African American female presented with a ruptured implant. The ruptured implant was a lower profile saline and was about ten years old. During the consultation, we discussed not only removing the implants but also what her goals were after her breast revision surgery. After setting the expectation, we decide to replace the old implants with high profile 425cc saline implants and overfill them to 465cc.

As you can see from the 7-week postop photos, she looks great in her clothing and she is very pleased with her size and shape.

Removal Of Ruptured Becker Breast Implants

Becker Breast Implant Removal

Recently, I had the opportunity to surprisingly operate on a patient who had a Becker double lumen ruptured implant.  This patient is a 52-year-old middle eastern female status post augmentation mammoplasty silicone gel implants per, with no identifiable records or patient’s accountability as to the type or size of her implants.

Intraoperatively, it was identified to be double lumen bilateral ruptured silicone and outer lumen saline ruptured implants.  Please see the photographs showing the ruptured Becker implant.  These are very unusual implants that were placed three decades ago.  There are very few of them still remaining in women throughout the world and it was a surprise finding in the operating room.  This patient desired to have the implants removed completely with explantation without reimplantation and drainage tube placement.  She also underwent breast mastopexy lift in order to reconstruct the shape of her breasts after losing a significant portion of her volume.

Becker implants were double lumen implants placed many years ago.  They had silicone inner lumen with an external secondary compartment of salt water.  The majority of these implants have been removed over the last 15 to 20 years and the few residual that are found require bilateral double lumen implant removal with removal of capsules often with calcification from leakage silicone.

Cleavage Enhancement After Previous Breast Augmentation

The female patient presented is 28 years of age who underwent a primary augmentation by a different surgeon a few years ago. During her consultation with me, she expressed that her breasts were positioned too high, and she had a poor-looking cleavage. After her examination and after discussing the expectations, we decide to perform a breast revision. During the breast revision surgery, we replaced her old implants with 700 cc saline high-profile implants. Using the high-profile implants provides a slightly wider base and an increase in upper pole fullness.

As you can see from the six-week post-op photo, the revision achieved a nice symmetrical balance and well-defined cleavage.

During this time of the year as summer approaches, a big part of my practice is performing primary breast augmentation procedures.
 
The patients presented to the left were both wanting to have a natural-appearing breast augmentation. Each patient was similar in age and size, and I find that the saline moderate-plus implants are a perfect fit. The moderate-plus implant is a mid-range implant that has a moderate width and a moderate amount of anteroposterior projection (in other words, a slight upper pull fullness).
 
Above are the before and after photos for both patients showing four weeks post op. As you can see, they have a nice fullness and a quite natural look.
