PEAR SHAPED (HIP) BODY TYPE WITH GYNOID FAT

Patients present to us with pear-shaped lower third gynoid appearance desiring to have liposculpture to proportion the lower third of their bodies to be symmetric with the upper and middle thirds. The example below shows a 21-year-old female presenting with severe pear-shaped gynoid appearance, disproportionally large fat along the iliac crest roll hip areas as well as the saddle bags and the anteromedial thigh. She is a good candidate for direct tumescent liposculpture technique in a bi-directional fashion, removing both the muffin-top as well as the saddle bags and the anteromedial thigh. She also had infragluteal fold or the infragluteal banana roll. Her operative photo shows a disproportionally increased amount of fat distributed along the lower third of her body, giving her a wide appearance. She underwent liposculpture using the tumescent technique with infiltrated fluid, sodium chloride, epinephrine and Lidocaine in the tumescent fluid, 1.3 liters of fluid were placed with a Klein needle and a 3 mm triple lumen Mercedes cannula was placed to 1 atmosphere of vacuum suction pressure, patient was placed under general anesthesia and liposculpture in a bi-direction of the iliac crest rolls was accomplished.

The next photograph shows her postoperative day one photo. Notice the softening of the iliac crest rolls of the hip area which has now smoothed down tremendously. She also has softening of the lateral thighs and the inner thighs which is quite obvious. Her thighs appear to be more proportionate with the middle third of her body. Patient will have sutures removed on day 7 and will undergo compression garment for six weeks. Tumescent liposculpture is very safe especially with women with gynoid appearances, who have disproportionally larger amounts of excessive fat in the lower third of their bodies.
