Plastic Surgery Consultations in Las Vegas

Dr. Linder was excited to see patients in Las Vegas over the weekend.  Patients most often arrive for breast reconstruction and revision surgery.  One of the consultations was with a patient specifically with severe scar tissue contracture with the left implant elevated superiorly and hardening with Baker IV capsular contracture and pain.  These can easily be fixed by reducing scar tissue, releasing the capsule and softening the pocket.  The implants can also be replaced from the low profile to high profile saline or silicone implants to increase projection and reduce lateral width. 

Other consultations that day also included mommy makeover, including breast implant lift, tummy tuck and liposculpture of the hips.  These patients present to Beverly Hills from Las Vegas for surgery, including breast augmentation, breast revision, breast lift, breast reduction, liposculpture, and abdominoplasty.


Great Candidate for Saline Implants

The below example is a patient showing with very narrow inframammary fold, minimal cleavage and bilateral breast hypoplasia.  Great candidate for saline implants placed under the muscle using high profile saline Allergan implants.  Due to the thickness of the muscle, the 500 cc, style 68 high profile Natrelle implants were placed through the periareolar approach and the subpectoral dual plane technique.  Preoperatively, she is hypoplastic, there is minimal upper pole fullness and the cleavage is approximately 4 cm along the intersternal distance.  Postoperatively, you can see at six weeks the implant position is excellent, there is good upper pole fullness and there is approximately 1.5 fingerbreadths of cleavage and the inframammary folds are even.  Because of the narrowness of this woman’s chest diameter, 12.5 cm inframammary fold distance, the high profile tapered implants are an excellent choice, allowing for upper pole fullness, good cleavage and reduction of lateral fullness or matronly appearance.  At six weeks she has softened appropriately and nicely.  She is now able to wear underwire brassieres and can continue workout and tissue expansion exercises. 

High profile saline implants are an excellent choice for younger patients under 25 years of age, who have thick muscle and bilateral breast hypoplasia as well as those who have narrow chest diameters.

Recovery After Breast Reduction

Dr. Linder very much enjoys the challenge of breast reduction surgery and performs reductions weekly. Breast reduction patients do extraordinarily well. Removing a significant amount of tissue from the breasts will enhance the patient emotionally and physically. Recovery after a breast reduction, usually requires certain steps. In Dr. Linder’s practice, the patients are either kept in an aftercare facility if they live a distance away or they often will be sent home with family members to take care of them overnight. If a patient undergoes a massive gigantomastic breast reduction, these will normally be performed in a hospital setting such as Cedars-Sinai Medical Center with a 23-hour overnight stay, usually approved by the insurance prior to the operation. Patients who are having anywhere between 500 and 1000 grams of breast reduction per side normally do not require hospitalization. Dr. Linder’s patients will have Jackson-Pratt drains placed for over 24 to 48 hours in order to remove fluid blood that may form around the inferior pedicle in order to prevent necrosis or problems with blood supply to the nipple areolar complex. We want to decompress the space around the pedicle that was created during the surgery, therefore, a 7 or 10 mm Jackson-Pratt drain will be placed for a minimum of 24 hours. Once the patients have left the recovery room, one-hour minimum in recovery by Federal law, they are asked to return the next morning at a set time. Once they return, the Bias dressings and the Jackson-Pratt drains will be removed. Brand new 4×4 gauze is placed and an athletic brassiere is then placed with the patient. The patients are asked not to remove any of these dressings or bras for the next seven days. They will be maintained on antibiotics, which is usually 750 mg of Keflex twice a day for one week and Vicodin ES p.r.n. pain, Zofran 5 mg ODT p.r.n. nausea and vomiting. Flexeril is usually not given because there is no submuscular dissection. Patients will be seen seven days later. During the first seven days, the patients will be asked to maintain their back and head elevated at 30 degrees in a bed or recliner as well as to keep the incisions dry and to maintain all dressings without changing them for seven days. If there is any oozing, they are asked to reinforce that with dressings. However, if there is significant oozing, the patients are required to immediately see Dr. Linder.

Beverly Hills breast reduction Before and After PhotoYour surgeon should be on-call 24 hours a day, 365 days a year for emergencies at any time and should have a Medicare State Licensed Hospital for emergencies in case the patient needs to be taken back to the hospital for bleeding or infection. This is an absolute must. The patients will then return seven days later, dressings are removed and a new sports bra or athletic brassiere will be placed with the patient and the dressings will be changed by the patient for the next seven days two to three times daily. Normally, the patients are off antibiotics and Vicodin and Tylenol can now be used for pain relief. We do not allow Advil, aspirin, Motrin, Excedrin, Ibuprofen or any aspirin-like products for the first three weeks. Sutures are removed usually on day 14 to 17 and the patients can then shower 48 hours later. Subsequently, the patients will use Kelo-cote twice a day, silicone gel spray to the incisions twice a day, once all sutures are removed. This helps to beautify the scar, reducing hyperpigmentation, keloiding, wide-spread scarring, the telangiectatic blood vessels that can be seen in the pink immature scarring. Patients will follow up two weeks later, now at four weeks, and again at eight weeks where normally the patients are ready for postoperative photography. Pictures are taken, lateral oblique and frontal views. At this time the patients may now exercise, aerobic and anaerobic, can wear underwire bras (usually by 4-5 weeks) and should wear athletic brassieres at night to maintain position of the breasts and to prevent recurrent gravitational descent.



The fashion this year at the 2011 Emmy Awards was definitely form fitting, tight body contouring gowns and dresses, specific to the most beautiful female forms.  On the Red Carpet, it was obvious that a multitude of celebrities from daytime prime time television as well as the Motion Picture Industry decided to show off their figures to its greatness.  Women had beautiful designer made dresses specifically accentuating their breasts and lower body shapes.  Red, tight-fitting dresses were also hot in the 2011 Emmy Red Carpet.  Examples include, red hot dresses with tight fitting body sculpting attire for Sofia Vergara, Nina Dobreu and Lea Michele, who all wore similar red hot color gowns that were form-fitting to accentuate the female form at its finest.

During the surgery, artificial breast implants are inserted into the breasts in order to increase their size and enhance their shape. Breast implant surgery can be performed alone or in conjunction with a breast lift for those looking to raise the breasts as well as increase their volume.

Dr. Linder most often uses smooth, round saline implants under the muscle because the shell of the implant doesn’t adhere to the capsule or the tissue and the implant can move more freely in the pocket. Additionally, because he inserts the implant through the periareolar approach, one often finds identifying the actual incision site afterwards to be quite difficult.

The incisions for one of Dr. Linder’s breast augmentations can be placed in a number of different locations. After making an incision, a pocket is created either under the breast tissue or under the pectoralis muscles. That pocket is made just large enough to accommodate the breast implant which is inserted into the pocket and then inflated, usually with saline, to give the best cosmetic result.

Dr. Linder positions breast implants as perfectly as possible by placing them under the nipple and down subpectorally using the dual plane technique. The lateral third retromammary subglandular and the medial two-thirds under the pectoralis major muscle. Placing the implants with two-thirds muscle coverage reduces visibility, palpability, allows for better contour of the upper pole of the implant and also reduces capsular contracture and hardening versus placing the implants subglandular.

It also is useful for detecting calcifications or breast masses on mammography, thereby improving radiographic diagnoses with mammography. Rippling is much less significant medially as well when there is pectoralis muscle covering the medial portion of the implant, especially on the thin ectomorphic built women. Patients who have developed capsular contracture and hardening require an open capsulectomy or open capsulectomy, implant exchange, as well as women who have had problems with placement of their original implants or malposition causing a severe deformity of the chest.

Criteria for Board Certification with the American Board of Plastic Surgery

In order to become a Board Certified Plastic Surgeon, the physician must first graduate from an accredited medical school, complete at least five years of additional training as a resident surgeon with a program accredited by the Accreditation Council for Medical Education or the Royal College of Physicians and Surgeons of Canada.  Therefore, it includes five years of residency training in all areas of surgery, two years devoted specifically to plastic and reconstructive surgery.  To be certified subsequently the doctor must specifically past both written and oral examinations after finishing a licensed plastic surgery fellowship.  To be a diplomate of the American Board of Plastic Surgery is a distinct honor.

The American Board of Plastic Surgery is one of 24 medical specialty boards recognized by the American Board of Medical Specialties.  It covers the full spectrum of plastic and reconstructive surgery, including head, neck, trunk and extremities.  Becoming a Diplomate of the American Board of Plastic Surgery signifies that certification has been met through requirements outlined.  The specific mission of the American Board of Plastic Surgery is to promote ethical, efficacious safe plastic surgery to the public, maintaining the highest of standards, including certification, education, examination and maintenance of certification.  A Board Certified Plastic Surgeon once again will have completed study in a medical degree in a recognized school and completed the training and an accredited residency program.  Each candidate will them pass written examinations subsequently and oral board examinations.  There are time limits of 10-year periods of which recertification will be required.
