Body Contouring After 3 Children

About six months ago, I performed a massive tummy tuck and breast lift on a patient with the intent to have a follow-up surgery to complete her full body contouring makeover. After having three children all by c-sections and undergoing significant weight loss, we agreed to reach her goal by having two surgeries. The second surgery entailed liposuction around the abdominal area and inner thighs as well as removal of excess skin (dog ears) that had bunched up from the tummy tuck procedure.

The first photo to the left was taken right before her first surgery. The middle photo was taken right before her second surgery (liposuction). The far right photo was taken three days post-op after her second surgery. The photo below is now four weeks post op. As you can see, her waist and stomach area look great, and she is extremely happy with her new shape.

WHAT TO DO WHEN YOUR BREAST LOOK DIFFERENT

Breast asymmetry can be associated with normally congenital, developmental or even traumatic or associated with breast cancer which may require breast reconstructive surgery causing breast asymmetry. Breast asymmetry is associated with one breast looking larger or smaller than the other. It can also be associated with one breast being saggier or having more ptosis than the other side. Correcting breast asymmetry can be very difficult, if not impossible. The correction of breast asymmetry may require creative thought, which may include different surgical procedures on one or both breasts in order to create similarities.

Technique #1: Using implants of different sizes to create symmetric breasts. This is most often associated with breast when the nipple areolar complex is above the fold and there is no degree of sagginess (ptosis).

Technique #2: Performing a breast reduction on a larger breast and simply a breast lift on the contralateral breast if both show sagginess, but one breast is significantly larger than the other.

Technique #3: Placing an implant on one breast and/or an implant and a lift on the other breast if the contralateral breast has a degree of sagginess or ptosis.

Technique #4: A breast reduction or lift on one breast alone to even it out with the left breast if the left breast shows no degree of sagginess and similar sizes in volumes can be created with a lift and implant on contralateral side.

Technique #5: A culmination of these multople procedures including breast lifts on both sides with different size implants if there is asymmetry of volume and/or both breasts show a significant degree of sagginess with a nipple areolar complex which may be below the inframammary fold grade III ptoris.

Primary Augmentation With Lift

I evaluated this 27-year-old female who requested to have a primary augmentation. During her examination, she mentioned that, after having two children, she was ready to get her pre-pregnancy shape back.

To the left, you can see her pre-op photo that shows severe skin laxity with a loss of upper-pole fullness of the breast. During her pre-op consultation, we also discussed her dark skin and her increased risk for hypopigmentation (loss of skin color). After discussing her options, we agreed on 420 cc saline high profile breast implants and performing a full mastopexy (breast lift). The mastopexy involves repositioning the breast and nipple/areola to a more youthful position and addresses the ptosis or sagging of the breasts.

The post-op photo is two weeks out, and she is very happy that she has regained her youthful shape. She will continue to follow my post-op instruction, which includes a compression bra and one-inch Steri-strips used at two-week intervals for a total of six weeks to reduce spreading of the scars.

The patient to the right is a 25-year-old female who was wanting to enhance the size and shape of her breasts. During her consultation, she explained her desire to reshape her breasts with a slight lift and increase the volume. After setting her expectation, we agreed to use  320cc Allergan high-profile saline breast implants for her breast augmentation surgery.

The post-op photo is 5 months out, and she is extremely happy with her fullness, size, and cleavage when wearing certain types of clothing.

She was very kind to leave a review on line, which she shared with me during her follow up earlier this week.

Bony Chest Wall Deformity Correction With Implant

The patient to the left is a 34 year old female that was requesting a breast augmentation. During the evaluation it was apparent she has a bony chest wall deformity and even a procedure like primary augmentation can be complicated due to this.  After listening to her expectations and discussing her deformity we agreed to move forward with her augmentation.  Choosing the right implant size and type is critical when dealing with this type of chest. During the augmentation we selected 340 cc Natrelle Inspira style SRX, which are a smooth shell and round shape. This implant due to its cohesive gel will help with shape, rippling and camouflage the chest area.

The post op photo is 8 weeks out, and the selections of the SRX implant has covered the bony chest, giving a natural look and excellent cleavage.

Breast Surgeries Prior To Holiday Season

As the holiday season approaches, my Beverly Hills practice always seems to be very busy when it comes to breast surgeries. This year is no exception, as my operating room staff and I performed seven breast surgeries yesterday. Those procedures included two breast implant revision surgeries due to ruptured saline implants, two breast implant revision surgeries due to capsular contracture, two implant removal and exchange procedures, and one primary breast augmentation.

Most patients elect to have this surgery during this time to take advantage of their holiday vacations for their recovery after the procedure. Recovery time varies due to the type of breast procedure ranging from two to three weeks. After surgery, patients are sent home with a Bias wrap and gauze in place. Patients should avoid heavy lifting and raising their arms above their shoulders for three weeks. Postoperative visits are scheduled over the next six weeks as needed.

One of the most rewarding procedures that I perform in my Beverly Hills practice is Breast Reduction or Reduction Mammoplasty. This is a procedure to remove excess breast fat, glandular tissue, and skin to achieve a breast size that is in better proportion with a woman’s body. It also alleviates the discomfort associated with overly large breasts and allows women to feel better.

I evaluated a 52-year-old female a few months ago who had breast hypertrophy (enlargement of breast tissue) and desired to have a breast reduction. Like many women with overly large breasts, she was having neck, back, and shoulder pain and was unhappy with their appearance. After listening to her expectations and agreeing on our approach, we set a date for surgery.

During her breast reduction surgery, I removed well over 1,000 grams of breast tissue from each breast and performed a formal mastopexy using a wise-pattern technique. The frontal and oblique view of the post-op photo is six months out, and she has gone from a 42 HH to a 42 DD. She is extremely happy with the outcome and is so excited to be able to wear a size large sports bra.

Breast Revision After Two Previous Breast Surgeries

I evaluated this 31-year-old female who requested to have a breast revision. During her examination, she mentioned that this was going to be her third breast surgery. She had had two prior breast surgeries from other surgeons in Southern California and was hopeful this would be her last.

The frontal pre-op photos to the left show capsular contracture (scar tissue) and malposition of the right implant. Due to minimal breast tissue, the left breast shows rippling from the implant as well as malposition of the implant and low nipple position.

The patient’s reconstruction or breast revision included removing the implants, nipple-areolar repositioning, and placing 445 cc SRX gel breast implants. Her post-op photo below is only 2 weeks out, but notice that she has no rippling, symmetrical balance, and nice fullness. She is very excited with the outcome of her procedure, and I will be seeing her a few more times over the next month to ensure an excellent result.
