Long-Standing Saline Implant Rupture – Breast Revision Case

Linder-removing-a-ruptured-saline-breast-implantThe other day at Summit Surgical Center in Beverly Hills, I performed a breast revision surgery on a 60-year-old woman from the Central Valley who had a long-standing rupture of a saline breast implant. While implant ruptures are not uncommon, this case was particularly complex due to the extended duration the rupture had gone untreated. During surgery, I discovered a dense, neurotic breast with evidence of a massive rupture. The implant—a McGhan 360cc saline implant—was dark brown in color and had likely been ruptured for over 10 years. This discoloration and capsule change are consistent with long-term leakage and chronic tissue response.

After removing the ruptured implant and performing a complete capsulectomy, I reconstructed the breast pocket, replaced the implant with a 500cc high-profile saline implant from Allergan™, and replaced the other implant. The patient desired restored volume and a more youthful contour, and the new implants provided a balanced and natural result. This case underscores the importance of regular monitoring and timely revision for aging breast implants—even saline. Long-standing ruptures not only distort breast shape but can also cause significant tissue changes over time.
